Dr. Jared Wayne Mack
DDS, Board Certified Oral and Maxillofacial Surgeon
Dr. Jared Mack grew up in Thatcher, Arizona located in the southeast corner of the state. He attended Eastern Arizona College where he played football for two years and then transferred to Brigham Young University where he received a Bachelor of Science in Biology. He completed his dental training at the University of Colorado School of Dental Medicine in Denver. While in dental school he received the Health Services Collegiate Program scholarship from the US Navy and began his Naval career.
He completed his residency training in Oral and Maxillofacial Surgery at the Naval Medical Center in San Diego, where he received extensive training in facial cosmetic, reconstructive surgery and dental implant surgery. After residency he was stationed aboard America’s Flagship the USS Ronald Reagan. After separating from active duty, he joined the US Navy Reserves and in 2020 was deployed in support of COVID to Camp Pendleton.
Dr. Mack is board certified and a Diplomate of the American Board of Oral and Maxillofacial Surgery, a member of the American Association of Oral and Maxillofacial Surgeons, American Dental Society of Anesthesia, the American Dental Association and the Arizona Dental Association.
He enjoys spending his free time with his wife and four children going on hikes and attending their various sporting and musical events.
Dr. Bentley Anderson
DDS, Board Certified Oral and Maxillofacial Surgeon
Dr. Bentley Anderson, DDS grew up in Mamaroneck NY, a town just 30 minutes northeast of Manhattan. He attended Utah State University and received a bachelor’s degree in Spanish graduating Magna Cum Laude. He also received two minors, one in biology and one in chemistry.
During his undergrad he was awarded the title of Undergraduate Research Scholar for his work in drug treatment of influenza and vaccinia virus. He obtained a grant for his research, had his finding presented internationally and published in the Journal of Virology.
Dr. Anderson attended the University of Utah School Of Dentistry as part of the inaugural class earning his doctoral degree in dental surgery. While in dental school he served as treasurer for the Utah Chapter of the American Dental Association (ASDA). He also participated in charity organizations including Give Kids a Smile and a dental charity trip to the Philippines.
Following dental school, he completed a residency training program in Oral and Maxillofacial Surgery at the University of Arizona College of Medicine-Phoenix. He served as chief resident at Banner University Medical Center from June 2020-June 2021. After residency, he elected to further his training and completed a fellowship in the treatment of facial trauma and temporomandibular joint reconstruction.
Empathy, compassion and a constant desire for improvement are the central components in his treatment philosophy. Quality patient care is his goal.

Dr. Anderson has been married to his beautiful wife, Adrea, for 13 years. They have 3 wonderful children. They are the driving force for his hard work and dedication.
Dr. Kevin Burns
DDS, Board Certified Oral and Maxillofacial Surgeon
Dr. Kevin Burns was born and raised in Mesa, Arizona. After graduating high school, he lived in Honduras for two years serving the people in numerous cities and pueblos as well as learning the beautiful Spanish language. It was a most humbling experience.
Dr. Burns attended Arizona State University and received a Bachelor of Interdisciplinary Studies Magna Cum Laude with emphasis in Biology and Small Business. He then attended Midwestern University College of Dental Medicine as a member of the 2008 inaugural class and earned the degree of Doctor of Dental Medicine graduating in 2012. During his time in dental school, he was elected to membership in the Honor Dental Society of Omicron Kappa Upsilon as well as awarded the Health Professional Scholarship Program by the United States Navy.
Following dental school, Dr. Burns served as a naval officer and dentist at Naval Medical Center San Diego completing a General Practice Residency followed by operational sea duty on the USS Bataan (LHD 5) which included a 9-month deployment to the Middle East. He then completed residency training in Oral & Maxillofacial Surgery at Naval Medical Central San Diego. After completing residency training, Dr. Burns returned to operational sea duty onboard the USS Harry S Truman (CVN 75), which included a 7-month deployment during the COVID pandemic and was one of two COVID free aircraft carriers operating in the world at that time. He was then assigned to another year of operational sea duty onboard the USS Abraham Lincoln (CVN 72). Dr. Burns completed his naval career December 31st, 2023 after 12 years of service grateful for many professional and personal growth opportunities as well as a great pride for the United States of America, and following the military service of his father whom served during the Vietnam War and his grandfather in World War II.
Dr. Burns is board certified and a Diplomate of the American Board of Oral and Maxillofacial Surgery, member of American Association of Oral and Maxillofacial Surgeons, and American Dental Association.
He married his wife Jill in 2004 and they have three wonderful daughters. Having spent so much time away from home and family you may hear him say, “It’s great to come home.”
